Comment: BDM91288 is a pyridylpiperazine-based bacterial efflux pump inhibitor (EPI) with improved drug-like properties compared to earlier inhibitors from the series such as BDM88855 [1]. It has potent activity against the resistance-nodulation-division (RND) family of efflux pumps of Enterobacteriaceae, binding to AcrB, the inner membrane component of the AcrAB-TolC efflux pump [1]. EPIs have potential clinical utility as adjunctive therapeutic agents for the treatment of infections caused by Gram-negative pathogens.

| BDM91288 has been shown to decrease the MIC of a panel of antibacterial agents (including levofloxacin, tetracycline, cefepime, ceftazidime, aztreonam, and meropenem) against Klebsiella pneumoniae in vitro [1]. The same study found that BDM91288 improved the in vivo efficacy of levofloxacin treatment in a mouse model of K. pneumoniae lung infection. |
