Comment: Trimethoprim is a synthetic diaminopyrimidine molecule with antibacterial and antiprotozoal activities. Mechanistically, it inhibits bacterial dihydrofolate reductase and disrupts the conversion of dihydrofolic acid to tetrahydrofolic acid which ultimately inhibits bacterial DNA synthesis. Sulfonamide drugs potentiate trimethoprim's activity.

Summary of Clinical Use ![]() |
Trimethoprim is primarily used to treat urinary tract infections, but can be used against infections by any susceptible aerobic bacterial species. It is not recommended as a treatment for anaerobic infections such as Clostridium difficile colitis. Fixed-dose formulations of trimethoprim plus sulfamethoxazole (Co-Trimoxazole) or dapsone are used to manage Pneumocystis pneumonia in HIV/AIDS patients. |
