Comment: Ethionamide is a thioamide compound with potent antibacterial activity against mycobacteria. It is used in the treatment of multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB). Ethionamide inhibits mycobacterial fatty acid synthesis by targeting InhA (an enoyl-acyl carrier protein reductase) [2], following intracellular activation, by the monooxygenase EthA, in Mycobacterium tuberculosis [1].

Ethionamide is indicated for the treatment of active tuberculosis when first-line therapies cannot be used because of resistance and/or intolerance. It is administered orally and in conjunction with other tuberculosis medicines. It has been used off-label as an alternative treatment for M. avium complex (MAC) infections and for multibacillary leprosy.
Ethionamide is on the World Health Organization's Model List of Essential Medicines and was given authorisation by the US FDA in 1965.
